Welcome to the School District of Lodi, a strong partnership of families, community and educators. We are a Wisconsin public school district serving over 1600 Pre-K thru 12th grade students from the communities of Lodi, Dane, Harmony Grove, Okee, and the Lake Wisconsin area.

Our website is designed to offer helpful communication about the aspects of our district that are important to the community. The site includes district-wide, school, and co-curricular information. Additionally, a majority of the teachers offer websites that can provide students and parents more in-depth information about the classroom learning activities.

Just as we seek continuous educational improvement through implementation of our strategic plan adopted in June of 2004, we also encourage you to contact us with any ideas that could increase the useful nature of this website.

This information is to guide you through the enrollment process at the school your child will attend. Please see individual school web pages for handbooks and school-specific information.

1. Contact the school your child will attend (see list on right).
2. Complete the "Student Release of Information" form (found on the District's Home page) to obtain records from previous district.	-Student Records Policy No. 533.1

4. 5. Complete Student Accident Insurance Waiver Form (available on registration day or in the school office).
6. Pay student fees as appropriate	-Student Fees Policy No. 527, Procedures
7. Meet with counselor to devise an educational plan for life-long learning and receive a tour of the school building (see list on right).
8. Contact Student Services if appropriate (student with special educational needs)

10. Medication Request/Consent Form (for all students who take medication at school).	-School Health Manual
